IA Chiropractic ("we", "us") is a Chiropractic clinic located at Hughenden Dr, Thornlie WA 6108. We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and our obligations as a registered health service provider.

We collect personal information needed to provide safe, appropriate care: your name and contact details, date of birth, health history, examination findings, treatment records, Medicare/health fund details where relevant, and appointment and payment records.

Booking and practice management data is processed through Cliniko, our secure practice management system. Our website uses Google Analytics to understand aggregate site usage; this does not identify you personally.

Your information is used to assess, diagnose and treat your condition, manage appointments and reminders, process payments and health fund claims, and communicate with other health practitioners involved in your care (with your consent).

We do not sell your information, and we do not use your health information for marketing without your express consent.

Clinical records are stored securely in encrypted, access-controlled systems. We retain health records as required by law, generally at least 7 years from your last visit, or until age 25 for records made when a patient was under 18.

You may request access to, or correction of, your health records at any time by contacting us on 0416 926 027. We will respond within a reasonable period. In limited circumstances access may be refused as permitted by law, and we will explain why.

We only disclose your information: with your consent (for example to your GP or another practitioner); where required by law; or where necessary to lessen a serious threat to life, health or safety.
